Five Ways SC Home Rx Simplifies Medication Management


South Carolina & Georgia House Calls (SC & GA HC) has been actively engaged in remote patient monitoring (RPM) since launching its program in October 2019. RPM technology allows daily monitoring of a patient’s vitals in the home. The vitals are then transmitted via a tablet to a central location to be analyzed by licensed medical professionals. The monitoring includes oxygen saturation, weight, blood pressure, temperature, and pulse. A glucose monitoring device is being evaluated for compatibility and is expected to be added to the kits in 2023.

RPM data is critical in developing and implementing appropriate care plans based on up-to-date patient information. By providing daily actionable data, RPM gives SC & GA HC practitioners an accurate real-time view of their patients. The information allows a practitioner to react to negative trends by promptly making adjustments.

RPM technology is a proactive approach to medicine designed to detect problems early, thereby reducing the chance of a more severe condition developing that could lead to hospitalization. Five hundred-four patients are monitored by SC & GA HC, with an additional 40 kits in transit and 47 kits in the installation process. Since the program’s inception, the hospitalization rate for RPM participants has remained below one percent.
