SC House Calls understands a person’s home is defined as where they reside, whether that is a Skilled Facility, Assisted Living, Independent Living or private residence. -
SC/GA House Calls provides every service you would expect from a traditional Doctor’s office visit directly to the patient in their home. Patients can choose to see a medical professional in person or via Telehealth. SC/GA House Calls provide services that include personalized care plans, medication management, ordering tests/labs, specialist referrals, follow-up appointments, remote patient monitoring, and much more.

No, we are a Primary Care Physicians group. Still, we work closely with Home Health across SC/GA by collaborating with them and their patients to provide the best possible care.

We bill the same way as a Doctor’s Office, through insurance. No hidden fees or travel charges are added to your bill. For example, if you have a $20 co-pay at your physician’s office, you would have a $20 co-pay for us.

We have an online portal that you may utilize to see and pay for your bill, you can mail a check once you receive your invoice via email or mail, or you can call one of our representatives in the billing department.

SC/GA promotes a proactive approach to healthcare. Through this healthcare model, we increase the quality outcomes and care a patient receives while decreasing the risk of unnecessary hospitalizations. We see our patients based on their needs a few times per month, either in person or via Telehealth, to ensure our patients are cared for appropriately.

We aim to have those orders taken care of for you. If there are orders that need to be handled, you can reach out to the account manager overseeing your building or the RN/MA for your facility.

Of course, but for our Independent and Assisted Living Communities, we highly recommend that we become your Primary Care Physician. This allows SC/GA House Calls to provide that proactive care and start to collaborate with the staff and medical team working on the campus where you reside. This will also allow for better communication, faster addressing of needs, and the ability to keep you or your loved one out of the hospital. For you, the resident, it allows you to stay in your home instead of having to leave and wait for long periods of time in a doctor’s office. Staying out of the waiting rooms will also decrease your chance of picking up any airborne illness.

A Care Team comprises a Physician, Nurse Practitioner, Pharmacist, Nurse, Social Worker, and Medical Assistant who oversees and directs the care of your loved one. This care team works together to provide more comprehensive care for yourself or your loved one.

You can still see your Specialist and one of our specialty services. We currently offer Behavioral Health and Psychiatric services, Cardiac services, Diabetic Management, Palliative Care, and Clinical Pharmacy Consults.

We also work with a mobile imaging company that provides x-rays and other imaging services to our patients in their homes.
