Pilot Program Seeks to Improve Healthcare Delivery to Assisted Living Communities

South Carolina House Calls (SCHC) is piloting a new care initiative for Assisted Living communities designed to provide the same patient care model used for private residences. The model has successfully eliminated unnecessary hospitalizations by increasing patient visits through both face-to-face and telemedicine visits. By seeing a patient sooner and more often after a medical event like an illness, hospitalization, or rehabilitation, positive outcomes increase exponentially.  

In addition to the emotional strain residents face when first entering an assisted living, they will often have socialization, nutritional, and deferred medical issues, including medication adherence. Using a provider during the transition helps establish a healthcare benchmark. A provider who performs an overall healthcare assessment, including pharmaceuticals and mental health evaluations, gains the information necessary to implement a proactive care plan.
