Your risk of non-payment or reduced payment in the first 30days will decrease in this partnership. Your quality scores will increase due to the more frequent aggressive visits and we will proactively prescribing/deprescribing medications and counsel on disease processes. Our physician is directly involved in the Care Plan Oversight and collaborates with the Nurse Practitioner to aggressively manage both acute and chronic patient disease.
